Photographic Permission

Prime Athletic Centre may wish to use photographs containing your my images for promotional, advertising and/or public relations purposes.

Such photographs may be included in Prime Athletic Centre’s program brochures, posters, Web-site and/or newspaper advertisements. Prime Athletic Centre will incur the full costs of such photography, including development and printing of relevant photographs. No individual’s names will be used in any print or electronic material except in the case of a video where the individual is being interviewed.

I give my permission for my photograph(s) to be used by Prime Athletic Centre for promotional, advertising and/or public relations purposes.

I acknowledge that all photographs, advertisements, Web site materials and related records and documents used in, arising out of or related to Prime Athletic Centre’s promotional, advertisings and/or public relations activities shall remain the exclusive property of Prime Athletic Centre who shall own all copyright.

I also waive any and all rights to any personality rights to Prime Athletic Centre for use on Prime Athletic Centre’s web-site or in any other promotional, advertising or public relations materials.
